چکیده

Background Left ventricular (LV) mechanical dyssynchrony has been proposed as a parameter to select patients for cardiac resynchronization therapy (CRT) [Bax et al JACC 2005]. Several recent studies have shown that placing the LV pacing lead in the most delayed regions yields a better response to CRT [Ansalone et al JACC 2002]. However, most imaging-based methods assess global LV dyssynchrony providing a single value for the entire LV. Regional maps of LV dyssynchrony are required for planning LV lead placement. The objective of this study was to develop a method to create a map of regional left ventricular mechanical dyssynchrony based on short-axis SSFP cine images.

Quantitative assessment of left ventricular mechanical dyssynchrony using cine cardiovascular magnetic resonance imaging: Inter-study reproducibility

OBJECTIVES To determine the inter-study reproducibility of left ventricular (LV) mechanical dyssynchrony measures based on standard cardiovascular magnetic resonance (CMR) cine images. DESIGN Steady-state free precession (SSFP) LV short-axis stacks and three long-axes were acquired on the same day at three time points. Circumferential strain systolic dyssynchrony indexes (SDI), area-SDI as we...

Left ventricular dyssynchrony can be observed via cine CMR with use of aortic valve timing

Background Cine CMR is the gold standard for evaluation of left ventricular function with high spatial and temporal resolution of the EKG-gated acquisition. Measurements of dyssynchrony have previously been performed using myocardial tagging to measure relative delays in peak shortening of opposing walls. We aim to utilize non-tagged images to quantify dyssynchrony in clinical patients. This is...

Ultrafast in-line interactive m-mode tool for quantification of left-ventricular (LV) septo-lateral wall motion (SLWM) from high-temporal resolution (6-12ms) cardiac cine steady-state free precession (SSFP) images

Introduction While conventional temporal resolution on the order of 30-50 ms is sufficient for calculating LV functional metrics such as global ejection fraction (EF), it is insufficient to accurately capture subtle regional wall motion dyssynchrony. A higher frame-rate cine imaging, e.g., 100 fps imposes an undue burden on the manual segmentation, precluding more sophisticated analyses, e.g., ...

Evaluation of automated contour detection to produce regional delay maps from high temporal resolution cine images in patients undergoing CRT

Background Recently, high temporal resolution short-axis cine images have been used to create left ventricular (LV) regional mechanical delay (RMD) maps in patients undergoing cardiac resynchronization therapy (CRT). Generating the endocardial segmentation required for RMD analysis is both time-consuming and subjective. A semi-automated segmentation method for time-continuous contour detection ...

Systolic Dyssynchrony Index derived from cardiac magnetic resonance imaging predicts left ventricular remodeling in heart failure patients undergoing CRT

Method 42 heart failure patients (38 male, ejection fraction 26 ±8.9%, NYHA 2.8±0.4), 21 with ischemic cardiomyopathy (ICM) and 21 with dilated cardiomyopathy (DCM) underwent a CMR prior to CRT implantation. Patients were scanned using 1.5T MR-scanner (Achieva, Philips Healthcare, Best, Netherlands) with either 32 or 5-element cardiac coil. Cine steady state free precession (cine-SSFP) images o...
